Job summary

Qualifications

• Licensure required as RN, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.

Responsibilities

• Coordinate the interdisciplinary care plan, discharge planning, and patient progress communication in the inpatient rehabilitation setting.
• Serve as liaison among patients, caregivers and the healthcare team to ensure timely and effective care transitions.
• Demonstrate strong communication and coordination skills; manage complex cases with medical and non-medical staff.
