North Florida Rehabilitation Hospital, part of Lifepoint Health, seeks a Licensed Case Manager for day shift part-time in Jacksonville, FL. You will coordinate interdisciplinary care, plan discharges, and communicate patient progress within inpatient rehab, serving as liaison among patients, families, and the care team.

Required licensure in RN, Social Work, or related field; CCM/ ACM preferred. At least 2 years in case management or social work in inpatient settings is desired; strong English

Formación

• Licensure required as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
• Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).

Responsabilidades

• Coordinate the interdisciplinary care plan, discharge planning, and patient progress communication within the inpatient rehabilitation setting.
• Serve as liaison among patients, caregivers, and the healthcare team to ensure timely and effective care transitions.
