Job Summary

This role is a core member of the integrated care team, providing short-term, individualized clinical interventions to members with behavioral health conditions such as substance use, depression, anxiety, schizophrenia, or bipolar disorder. The focus is on trauma-informed care, harm reduction, and motivational interviewing to promote behavioral change and adherence to treatment plans.

The BHC serves as a behavioral health SME within an interdisciplinary team, assessing members through a biopsychosocial approach and developing person-centered treatment plans with measurable goals. Responsibilities include providing individual and group therapy, post-hospital follow-up, and collaborating with primary care and psychiatric providers.

Key Responsibilities
  • Provide therapy, assessments, treatment plans, clinical documentation, and progress tracking.
  • Conduct crisis intervention and de-escalation as needed.
  • Serve as a behavioral health SME, offering recommendations and referrals.
  • Utilize evidence-based approaches such as trauma-informed care, harm reduction, and motivational interviewing.
  • Maintain communication with care team members and document care in electronic health records promptly.
  • Develop and support treatment plans for chronic mental illnesses and promote medication adherence when appropriate.
  • Assist with scheduling, appointment reminders, and overcoming attendance barriers.
  • Develop group therapy curricula for various topics.
Minimum Qualifications
  • Licensed in the state of practice with 3+ years of experience in behavioral health.
  • Licenses such as LCSW-C, LPC, LPCC, LCPC are acceptable.
  • Experience in community mental health or inpatient settings preferred.
  • Experience with complex populations and telehealth preferred.
  • Strong clinical judgment, computer skills, and communication skills.
  • Active CPR certification.
Working Conditions

Professional office environment, with potential for remote work based on business needs and health protocols.

Physical Requirements
  • Effective communication and ability to remain stationary for periods.
  • Operate office equipment and occasionally move objects up to 20 lbs.
