Indiana Health Centers, Inc. Bedford seeks a compassionate Licensed Behavioral Health Professional to join our outpatient team.

You will provide integrated behavioral health services within the Primary Care Behavioral Health model and collaborate with a multidisciplinary group to support patients in Owen County. The position offers a 4-day work week, flexible schedule, dedicated case manager, administrative support, ongoing training, generous PTO, and a robust benefits package that includes

Qualifications

• Master’s degree in social work required.
• Indiana licensure (LCSW/LMFT/LCMHC) required or able to obtain.
• BLS certification (AED included) required.

Responsibilities

• Provide comprehensive assessment and diagnosis of behavioral health patients.
• Assist providers in recognizing and treating mental, psychosocial, and substance use disorders.
• Develop treatment plans and support goal achievement.
• Evaluate crises and refer appropriately.
• Maintain active communication with PCPs during clinic hours.
• Deliver same-day and scheduled initial interventions.
• Conduct brief follow-up visits for selected patients.
• Participate in meetings supporting the integrated care model.
• Assist with care management processes and patient education.
• Teach patients and families care, prevention, and treatment techniques.
• Monitor the program and recommend improvements.
