Responsibilities

• Provide individual, group, and family counseling services based on client needs and scope-of-practice requirements.
• Complete or contribute to behavioral health and SUD assessments within scope of practice.
• Develop individualized, person-centered treatment plans with measurable goals.
• Facilitate structured groups on recovery skills, relapse prevention, and coping strategies.
• Evaluate progress and adjust treatment approaches as needed.
• Identify relapse indicators and safety risks that may impact participation.
• Coordinate care with medical providers, case managers, and community resources.
• Maintain accurate documentation including progress notes and treatment plans.
