Job summary

Carle Health seeks a licensed Counselor to join our patient-centered behavioral health team. You will provide individual, family, and group counseling, assess psychosocial needs, and develop treatment plans. You will work closely with physicians and nurses to support integrated care and document encounters in the EHR.

Responsibilities include crisis intervention, safety planning, patient education, and participation in quality improvement. Strong collaborative and ethical practice is required.

Qualifications

• Licensed counselor with eligibility for licensure in state of practice.
• Experience with evidence-based therapies and psychosocial assessments.
• Comfort collaborating with physicians and other health professionals.

Responsibilities

• Provide individual, family, and group counseling using evidence-based practices.
• Conduct psychosocial assessments and develop patient-centered treatment plans.
• Collaborate with physicians, nurses, and interdisciplinary team members on care plans.
• Document all clinical encounters in the electronic health record accurately and timely.
• Provide crisis intervention and safety planning as needed.
• Educate patients and families on coping strategies and available community resources.
• Participate in case conferences and quality improvement activities.
• Maintain compliance with ethical, legal, and regulatory standards.
