Job summary

Community Health Center of Middletown seeks a Behavioral Health Clinician II to perform diagnostic psychosocial evaluations and develop care plans with clients and their families. The role offers individual, family, or group therapy in an outpatient setting.

As a licensed independent provider, you will collaborate with psychiatric and medical teams, document interventions, and may supervise unlicensed staff. State standards and Joint Commission guidelines guide every aspect of care.

Qualifications

• Licensed independent provider with relevant clinical credentials.
• Minimum two years of outpatient experience, including work with children and/or adults.
• Fluency in English; bilingual Spanish helpful.

Responsibilities

• Perform diagnostic psychosocial evaluations and develop care plans with clients and families.
• Provide individual, family, or group therapy as appropriate in outpatient clinics.
• Collaborate with an interdisciplinary team and coordinate with psychiatric/medical providers.
• Complete timely documentation per state regulations and Joint Commission standards.
• Supervise unlicensed staff or students as assigned.
