Job summary

Horizon Blue Cross Blue Shield of New Jersey is seeking a Behavioral Health Care Manager to perform care management duties and guide transitions across the continuum of care. This role assesses clinical needs, coordinates services, and collaborates with members, families, providers, and hospitals to ensure appropriate, cost-effective care.

Qualifications include a high school diploma, bachelor’s degree preferred, and at least two years of behavioral health clinical experience; active NJ

Qualifications

• High School Diploma/GED required.
• Bachelor’s degree preferred or relevant experience in lieu of degree.
• Requires a minimum of two (2) years behavioral health clinical experience. Behavioral health experience in managed care setting strongly preferred.

Responsibilities

• Assesses patient’s behavioral health needs against guidelines to ensure appropriate inpatient level of care.
• Evaluates the necessity, appropriateness and efficiency of behavioral health medical services and procedures.
• Coordinates and assists in implementation of plan for members.
• Monitors and coordinates services outside the network and negotiates fees; coordinates with patient, family, physician, hospital and other external customers.
• Coordinates delivery of high quality, cost-effective care across the continuum of care.
• Documents accurately and comprehensively based on standards of practice and policies.
