Job summary

Horizon Blue Cross Blue Shield of New Jersey is seeking a field-based Care Manager to assess and coordinate services for members, guiding them through care plans with families, physicians and the healthcare team. You’ll ensure high-quality, cost-effective care while applying clinical guidelines across settings.

Travel locally is required; you’ll document thoroughly, advocate for members, and participate in quality improvement to optimize outcomes and resource utilization within this program.

Qualifications

• Bachelor degree in health-related or behavioral science field.
• 3–4 years broad clinical experience.
• 3 years in home care, discharge planning, or case management.
• 2 years in the managed care industry.
• Active Unrestricted NJ Social Work license preferred.
• Local travel and possible on-call work related to the program.

Responsibilities

• Assist with implementation and facilitation of services to members.
• Coordinate care with member/family, physician and healthcare team.
• Deliver high quality, cost-effective care per practice guidelines.
• Advocate for members across sites to coordinate resources.
• Document accurately per standards and policies.
• Field-based and telephonic case management activities.
