Job summary

North Country Healthcare is seeking a Substance Use Disorder Counselor/Clinician to provide evidence-based assessment, counseling, crisis response, and recovery support within a New Hampshire Doorway program and integrated healthcare setting.

You will screen, assess, and coordinate care across primary care, emergency, and community workflows, partnering with patients, families, and community organizations to develop individualized plans and improve engagement in treatment.

Qualifications

• Associates in Substance Abuse Counseling, or equivalent in related field; Masters preferred.
• LADC must be able to practice in New Hampshire.
• Certification: BLS/ACLS/PALS per position; Heartsaver CPR AED for nonclinical staff.

Responsibilities

• Provide evidence-based SUD assessment, counseling, crisis response, and care coordination.
• Deliver individual, family, and group counseling using trauma-informed approaches.
• Coordinate referrals, follow-up, and services across continuum of care.
• Document screenings, assessments, interventions, and outcomes in EHR timely.
• Support Doorway and SBIRT services across primary care, emergency, and community workflows.
