The LVN provides direct patient care and clinical support in a nonprofit outpatient clinic focused on LGBTQ+ primary care and HIV services.

Aligned with Ryan White Program standards, the LVN supports linkage to care, retention, adherence, and viral suppression while delivering trauma-informed, patient-centered care in a culturally competent environment.

Qualifications

• Current and unrestricted State of Texas LVN license.
• Graduate of an accredited vocational nursing program.
• Current Basic Life Support (BLS) certification.
• Experience utilizing electronic health record systems.
• Knowledge of clinical workflows, medical terminology, infection prevention, and patient education principles.
• Passion for health equity, diversity, and inclusion.

Responsibilities

• Prepare patients for examinations, procedures, and treatment.
• Obtain and document vital signs, chief complaints, health histories, medication reconciliations, and screenings.
• Assist providers during examinations, procedures, and treatments.
• Administer medications, immunizations, injections, and treatments as ordered by licensed providers and within LVN scope of practice.
• Perform phlebotomy, specimen collection, laboratory preparation, point-of-care testing, and EKGs as required.
• Provide wound care and other nursing interventions within scope of practice.
• Monitor patients for adverse reactions and changes in condition and report findings to providers.
• Maintain timely and accurate documentation in the electronic health record (EHR).

Job description

The Licensed Vocational Nurse (LVN) provides direct patient care, clinical support, care coordination, andpatient education services within a nonprofit outpatient clinic specializing in LGBTQ+ primary care, HIV

specialty care, Ryan White Program services, sexual health, STI prevention and treatment, PrEP/PEP services, and gender-affirming care.

As a member of a multidisciplinary care team, the LVN supports the clinic's goal to improve health outcomes and reduce health disparities among LGBTQ+ individuals, people living with HIV, and other medically underserved populations.

The LVN delivers culturally responsive, trauma-informed, and patient-centered care while supporting Ryan White Program standards related to linkage to care, retention in care, treatment adherence, and viral suppression.

Duties & Responsibilities

• Prepare patients for examinations, procedures, and treatment.

HIV Specialty Care and Ryan White Services

• Support comprehensive HIV medical care for people living with HIV.
• Assist providers in monitoring antiretroviral therapy (ART), laboratory testing, and treatment outcomes.
• Educate patients regarding HIV disease management, medication adherence, laboratory monitoring, viral suppression, and disease prevention.
• Support rapid antiretroviral therapy initiation and same-day treatment protocols when appropriate.
• Conduct patient outreach and follow-up regarding laboratory results, medication refills, missed appointments, and treatment adherence concerns.
• Assist with HIV-related laboratory monitoring including viral load and CD4 testing schedules.
• Reinforce provider treatment plans and medication instructions.
• Support newly diagnosed patients through linkage-to-care processes.
• Participate in interventions designed to improve retention in care and viral suppression.

LGBTQ+ Aff...ing and Gender-Affirming Care

• Provide culturally competent and affirming healthcare services to LGBTQ+ patients.
• Consistently utilize patient-selected names, pronouns, and gender identities in all interactions and documentation.
• Support gender-affirming care services through patient education, laboratory coordination, medication administration, and follow-up activities.
• Create and maintain a welcoming, inclusive, and nonjudgmental care environment.
• Demonstrate cultural humility and sensitivity when interacting with patients from diverse backgrounds and experiences.

Administrative Responsibilities

• Process prescription refill requests according to clinic protocols.
• Assist with referrals, prior authorizations, laboratory coordination, and specialty appointment scheduling.
• Manage provider messages and clinical communications as assigned.
• Maintain accurate records and complete documentation within established timeframes.
• Attend required staff meetings, trainings, continuing education programs, and quality management activities.

Quality Improvement & Compliance

• Participate in quality improvement initiatives focused on HIV viral suppression, retention in care, PrEP persistence and adherence, Preventive health measures, patient engagement, and health equity outcomes.
• Ensure culturally competent care delivery aligned with LGBTQ+ health best practices.

Qualifications & Skills

Preferred Qualifications

• Experience with community health centers, FQHCs, nonprofit healthcare settings, or public health organizations.
• Experience in HIV specialty care, sexual health, PrEP, PEP, or STI services.
• Experience serving LGBTQ+ communities.
• Familiarity with electronic health records (EHR) systems (e.g., eClinicalWorks).
• Bilingual (English/Spanish) is a plus.
