This grant-funded position results from a collaboration between Chicago Commons and the Center for Better Aging. The Licensed Practical Nurse will oversee case management responsibilities for the Welcome Home program, including conducting initial assessments and making referrals for clients discharged from the St. Bernard Emergency Room.

In accordance with the Nurse Practice Act and regulatory standards, the LPN will provide patient care and supervise non-clinical staff to ensure effective case management services. This includes utilizing assessment tools, making necessary referrals, coordinating services, and advocating for clients to enhance their health outcomes.

This home care nursing position requires strong clinical assessment, critical thinking, communication, and interpersonal skills. Travel to clients’ homes is an essential part of fulfilling the job responsibilities.

Education & Certification Requirements
  • A current Illinois licensed practical nurse.
  • Minimum of 5 years of nursing experience in a clinical setting.
  • Prior home care experience preferred.
  • Graduate of a state approved nursing school.
Experience & Training Requirements
  • Strong interpersonal and customer service skills.
  • Strong foundation in clinical assessment.
  • Travel is required.
  • Ability to communicate effectively in writing, verbally, and electronically.
  • Ability to work as a member of an interdisciplinary team, including healthcare providers.
  • Ability to work independently, be self-directed, and adapt to unpredictable circumstances.
Desired Outcomes

Improve Commons referrals by 10-15%.

Improve the delivery of quality care through client and staff education.

Improve internal referral communication by developing standards that are clinically proven.

Essential Job Requirements & Duties
  • Makes initial evaluation visits to evaluate client’s needs.
  • Initiates and coordinates program services for clients.
  • Attend and participate in clinical huddles and meetings related to the program.
  • Supervise non-clinical staff within the program.
  • Initiate appropriate preventive services as necessary for clients.
  • Maintain patient assessments and confidentiality in accordance with HIPAA regulations.
  • Facilitates regular meetings with program staff.
  • Promote, and comply with safety standards in a client's home.
  • Ensures all documentation is consistently legible, accurate, and completed in a timely manner.
  • Complies with Chicago Commons handbook, policies and procedures.
  • Consults with a Registered Nurse to ensure compliance and regulatory standards.
  • Attends all compulsory and recommended training and meetings.
  • Provide education to patients and their families.
  • Performs data collection as directed for trend analysis and program development.
  • Serves as a liaison between clients and external agencies.
  • Support the Adult Day Service team at Chicago Commons as needed.
  • Performs additional duties commensurate with job tasks.

While performing the duties of this job, the employee is regularly required to speak English, specifically, to communicate both in person and over the telephone. The employee must regularly be able to read English proficiently. The employee must regularly operate a word-processing system and be able to read, produce and transmit data on such a system. The employee frequently is required to stand, walk, reach with hands and arms, and stoop, kneel, crouch, or crawl. The employee must be able to lift and/or move up to 20 pounds occasionally. Specific vision requirements include the ability to see at a close range.
