Job summary

Monarch in New Bern, NC is seeking a Licensed Practical Nurse (LPN) to provide medical services under RN supervision in a residential setting. You will administer medications, perform nursing assessments, and support daily living activities while coordinating with physicians and pharmacies.

The role requires an LPN license in the state and an associates/HS diploma. Schedule is Monday–Friday with on-call rotation; 40 hours per week with on-site duties.

Qualifications

• Associates degree in Nursing or equivalent.
• High School Diploma required.
• Licensed Practical Nurse (LPN) license valid in the state.

Responsibilities

• Provide routine and emergent medical care as per state regulations and service definitions.
• Administer medications to residents and supervise medication administration by staff quarterly.
• Schedule physician appointments and coordinate medical scheduling.
• Maintain Medication Administration Records and ensure proper transcription.
• Ensure physician order updates for prescribed and OTC meds every six months.
• Coordinate with pharmacies, physicians, and labs to meet medical needs safely.
• Conduct routine medication counts and prevent counting errors.
• Support self-administration programs and participate in compliance surveys.
• Maintain medical supplies and ensure OSHA compliance.
• Assist individuals in exercising their rights and promote independence in ADLs.
• Provide transportation to participants as required and attend training sessions.

This person will provide nursing services including administration and monitoring of medication, nursing assessments and interventions.**What You'll Do:**• Provide routine and emergent medical care as specified by State Regulations and service definitions. • Administer medications to individuals in a residential setting and review all staff certified in medication administration a minimum of quarterly in order to insure best practices are followed. • Schedule necessary physician appointments and monitor ongoing scheduling of medical appointments for individuals receiving services to ensure proper documentation is completed following such appointments. • Update monthly Medication Administration Records ensuring all medications are properly transcribed onto a form and maintain them in accordance with program standards and individual needs. • Ensure quarterly and six-month physician order updates occur for prescribed and over-the-counter medications. • Coordinate efforts between pharmacies, physicians, laboratories, etc., in order to ensure medical needs of individuals being supported are met and coordinated safely. • Conduct routine medication counts in order to ensure accurate counts have been documented and that no medication counting errors have occurred. • Develop and monitor programs to help individuals being supported perform self-administration. • Participate in preparing annual compliance surveys conducted by the Division of Facility Services. • Maintain an adequate supply of medical supplies. • Adhere to person-centered activity schedules as assigned. • Ensure a safe environment for the individuals being supported and ensure OSHA compliance is maintained. • Assist individuals in the execution of their rights. • Promote independence in all areas of activities of daily living. • Drive and travel, arrange for, or provide transportation to participants as required. • Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements. • Demonstrate knowledge of and comply with all agency policies and procedures, as well as service definitions related to specific program areas. • Complete all other relevant responsibilities as assigned by the supervisor.
