Job summary

Villa Saint Joseph in Pennsylvania seeks an LPN to oversee resident care in a long-term care setting. You will provide treatments, administer medications, and work with physicians and the DON to determine levels of care.

Qualifications

• Must possess an LPN license in PA.
• Must be over 18 years of age.
• Must have been licensed in the BPOA PA Licensing System.
• CPR/AED certification required.
• Experience in long-term care.

Responsibilities

• Oversee the care of residents and all the components of that care.
• Provide medical treatments as ordered.
• Responds to immediate family member concerns regarding resident care.
• Administers medications in a safe and effective manner, responsible for monthly medication administration records.
• Daily and monthly recaps of resident orders.
• Participates in updating care requirements for each resident with the DON to determine level of care.
• Collaborate with health care professionals via phone or tele-med.
• Record each medication delivery from Pharmacy during the shift and ensure accuracy.
• Communicate with physicians and other providers regarding care issues, scheduling appointments and transportation.
• Oversee the Nursing Assistants.
• Respond to emergencies according to policy and clinical judgement.
• Develop and implement care plans and update as needed.

• Communicates with physicians, and other professional service providers, regarding resident care issues and the scheduling of appointments and transportation.
• Oversee the Nursing Assistants.
• Responds to any immediate need or emergency in the building according to policy and clinical judgement.
• Able to develop and implement care plans and update as needed.

Primary Requirements:

• Must possess an LPN license in the state of PA.
