Professional Healthcare Resources in Virginia is seeking a compassionate LPN to join our home health team. The ideal candidate will provide one-on-one in-home care, perform initial and ongoing assessments, monitor vital signs, administer medications as prescribed, and educate patients and families on care strategies.

The role requires a current LPN license, CPR certification, reliable transportation, and the ability to travel locally to patient homes.

Formación

• Must be a graduate of an accredited nursing program (Diploma/Associate/Bachelor’s) or vocational nursing.
• Licensed as LPN or RN in the state.
• CPR certification current.

Responsabilidades

Job Summary

We are looking for a compassionate LPN to join our growing team! The right candidate has a can-do attitude, a friendly demeanor, excellent interpersonal skills, and the ability to provide medical support to our clients in their homes.

Responsibilities

• Visit patients in their homes for one-on-one visits
• Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients
• Assess patients’ conditions and chart observations, and perform evaluation tasks including vital signs and medication review
• Administer medication as prescribed by the patient’s physician
• Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life
• Provide effective communication to patients/clients, their family members, team members, and other health care professionals

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing
• Current state license as an LPN/RN
• Proof of current CPR certification
• Compassionate, friendly demeanor
• Excellent communication skills
• Current driver’s license and reliable transportation
• Ability to work a flexible schedule and travel locally
