Description
Riverside Healthcare is seeking a motivated and compassionate Licensed Practical Nurse (LPN) with a license pending to join our Assisted Living and Memory Care team. This position is ideal for recent LPN graduates or those awaiting licensure. The LPN will work under the supervision of an RN to provide high-quality care to residents, including those with memory impairments such as Alzheimer’s and dementia. The role offers an excellent opportunity to gain hands-on experience in a supportive environment while ensuring the well-being and safety of our residents. Demonstrates flexibility with assignments within professional scope/duties/licensure.
Essential Duties
- Resident Care:
- Assist with assessing residents' health and provide care according to their service plans.
- Monitor residents’ conditions, report changes to supervising nurses.
- Provide basic nursing care such as wound care, dressing changes, and vital sign monitoring under supervision within scope of practice.
2. Medication Assistance:
- Assist with the administration of medications under the supervision of a licensed nurse.
- Monitor residents for adverse reactions and report any concerns immediately.
3. Support to CNAs/NA’s:
- Work collaboratively with Certified Nursing Assistants (CNAs)/Nursing Assistant (NAs) to ensure the delivery of high-quality care.
- Provide guidance and support to CNAs/NAs in their daily tasks and ensure care is provided according to the residents' service plans.
4. Documentation:
- Accurately document care provided, changes in residents' conditions, and any communication with families or healthcare providers.
- Ensure all documentation complies with health system policies and HIPAA regulations.
5. Resident Interaction:
- Build positive relationships with residents and their families.
- Engage residents in activities that promote their physical and emotional well-being.
6. Collaboration:
- Work closely with supervising nurses, healthcare providers, and other team members to deliver coordinated care.
- Participate in service plan meetings and contribute to discussions about residents’ care needs.
7. Organizational Compliance:
- Adhere to all applicable federal, state, and local regulations, including those related to infection control, safety, abuse & neglect and resident privacy (HIPAA).
- Mentor and guide less experienced nursing staff and assist with training new employees.
- Promote a positive and supportive environment for both residents and staff.
- Regularly complete all assigned training modules and courses within the Learning Management System (LMS) to stay up to date on required skills, policies, and industry standards.
Non-essential Duties
- Resident Activities: Support resident engagement in activities and events to foster a positive and stimulating living environment.
- Administrative Duties: Assist with administrative tasks such as inventory management, record keeping, and scheduling as needed.
- Professional Development: Engage in workshops, training sessions, and other educational opportunities to stay current with best practices in assisted living and memory care.
Requirements
Preferred Experience
- Experience in a healthcare setting preferred, especially in long-term care or skilled nursing.
- Strong communication and interpersonal skills, attention to detail, ability to work effectively under supervision, and proficiency in basic nursing procedures.
- Compassionate, patient-centered approach, with a strong commitment to professional development and adherence to ethical nursing practices.
Required Licensure/Education
- Graduate of approved school of nursing
- Proof of passing the NCLEX-PN
- Submission/payment to IDPFR for Illinois LPN license
- Current BLS
