Vertis Therapy is looking for a passionate and detail-oriented Physical or Occupational Therapist for our Lead Therapist position in Ft Wayne, IN at our beautiful Assisted Living Facility. Primary Job Function

The Lead Therapist’s primary responsibility is to provide leadership to the therapy team, including managing the day-to-day operations of the therapy department, supervising therapy staff, and coordinating department activities. The secondary responsibility is patient care. The Lead Therapist demonstrates high performance in Vertis core values, leadership, team development, customer service, and department operations. The Rehab Director works closely with the Regional Manager to ensure that Vertis standards are maintained and resources are efficiently utilized.

Primary Responsibilities

  • Ensure rehabilitation services are provided in compliance with Vertis and facility policies, procedures, and regulatory standards.
  • Coordinate resident screening programs (PT / OT / SLP).
  • Advocate for meeting the rehabilitation needs of residents.
  • Participate in care plan and departmental meetings.
  • Act as a liaison between the therapy department, administration, and nursing staff.
  • Ensure accurate and timely month-end billing.
  • Coordinate therapist schedules and arrange prn staffing as needed.
  • Communicate PT / OT / SLP schedules and changes to administration and nursing staff.
  • Coordinate the execution of the Restorative Program with nursing staff and the therapy department.
  • Prepare the Rehab Department for deficiency-free State Surveys and represent the department during the survey process.
  • Complete Vertis guide to survey twice annually.
  • Train staff in Vertis and facility policies and procedures.
  • Identify therapy needs of the resident population.
  • Lead the therapy team in implementing clinical programs.
  • Communicate pertinent information from Vertis management and facility management to the therapy team.
  • Assist Regional Manager in performance reviews, applicant interviews, and disciplinary actions.
  • Assist in the orientation of new employees and contract therapists.
  • Support team development by encouraging growth, recognizing accomplishments, and fostering a positive work environment.
  • Perform other duties as assigned by the Regional Manager.
  • Analyze team productivity levels and facilitate achievement of company expectations.
  • Assist in analyzing key operational data to support company success.
  • Provide direct patient care, applying discipline-specific principles, methods, materials, and equipment used in this facility.

Requirements

Minimum Qualifications / Requirements

  • Master of Science degree in Physical Therapy or Occupational Therapy, or Bachelor of Science degree in either, with at least five years of practice experience.
  • Minimum one (1) year experience in a rehabilitation setting serving the same age and patient population as this facility.
  • Excellent communication and interpersonal skills.
  • Proficient computer skills for documentation and required job duties.
