Qualifications

• Bachelor’s degree in Social Work or related field required.
• Completion of state-required case management/service coordination course within six months of employment.
• At least 1 year post‑Bachelor’s experience with adults or ACT population in a human services setting preferred.

Responsibilities

• Conducts and documents comprehensive assessment of client and/or client’s needs.
• Develops client service plans consistent with assessment.
• Arranges delivery of needed services identified in assessment.
• Assists client in accessing needed services via referrals or linkages.
• Monitors progress by tracking appointments, referrals, and reassessments.
• Performs advocacy activities on behalf of client to expedite entitlements.
• Maintains case records per standards and regulations.
• Provides case consultation with service providers/collaterals.
• Communicates with supervisor about case management, resources, and development of new resources.
• Provides psychosocial education to clients and/or families.
• Performs crisis assistance and emergency referrals.
• May serve as Primary Case Coordinator (PCC) depending on skill and autonomy.
