Job summary

Human Services Administration Organization (HSAO) in Pittsburgh, PA is seeking a LEADSpecialist to coordinate care for individuals and families with behavioral health or justice-related needs. You will provide case management, develop service plans, and advocate for clients in a collaborative, person-centered team.

You will work in a full-time, hybrid role with some remote administrative tasks, field visits, and coordination across county agencies.

Qualifications

• Bachelor’s degree in social work, psychology, education, nursing, criminal justice, or related field; master’s preferred.
• Minimum of two years in case management or behavioral health.
• Knowledge of mental health and addiction treatment principles.
• Strong organizational skills with ability to manage a caseload and deadlines.
• Excellent communication and interpersonal skills with cultural competence.
• Ability to work both independently and in a team.

Responsibilities

• Serve as the primary contact and advocate for individuals and families.
• Develop individualized service plans based on assessments and update regularly.
• Provide face-to-face support, meeting at least once per month.
• Assist clients in navigating the criminal justice system, including court proceedings and treatment services.
• Coordinate with behavioral health providers to ensure access to mental health, drug, and alcohol services.
• Facilitate interdisciplinary team meetings with stakeholders and partners.
• Advocate for clients' access to resources and address systemic barriers.
• Collaborate with treatment providers to develop discharge plans and post-release services.
• Serve as liaison between clients, providers, and systems.

Position Overview:

LEAD is a proven tool to better respond to crime and disorder caused by people whose conduct stems from mental illness, substance use disorder, or chronic poverty by diverting them out of the jail cycle and into community-based care. LEAD provides long-term case management support for individuals with unmet behavioral health or substance abuse needs.

Key Responsibilities:

Client Support and Case Management:

• Serve as theprimary contact and advocatefor individuals and families, ensuring that services align with their needs and goals.
• Develop individualizedservice plansbased onassessments andupdate them regularly to reflect progress and evolving needs.
• Provideface-to-face supportto clients, meeting at least once permonthor as needed.
• Assistclients in navigating thecriminal justice system, including court proceedings, probation/parole requirements, and treatment services.
• Coordinate with behavioral health providers, ensuring access tomental health, drug, and alcohol servicesand other communitysupports.

Collaboration and Advocacy:

• Facilitateinterdisciplinary team meetingswith stakeholders including families, healthcare providers, probation officerswhenappropriate,and any other entities who may be a part of someone’s larger treatment team.
• Actively advocate for clients' access to needed resources and provide feedback to address systemic barriers.
• Collaborate withtreatment providersand other community partners to developdischarge plansand post-release services.
• Serve as aliaisonbetween clients, providers, andsystems, ensuring smooth communication and care coordination.

Documentation and Compliance:

• Maintainaccuraterecords of interactions, includingservice plans, case notes, and assessments, in electronic health records.
• Ensure compliance withHIPAAand other confidentiality policies.
• Participate in program audits and quality reviews to ensure that services meet agency and regulatory standards.

Training and Development:

• Participate inongoing trainingto stay current with best practices in mental health, substance use, and criminal justice services.
• Offer support and mentorship tonew staff, fostering a collaborative learning environment.

Required Skills and Qualifications:

• Bachelor’s degree in social work, Psychology, Education, Nursing, Criminal Justice, or a related field (master’s degree preferred).
• Two years of experiencein case management, behavioral health, or a related field (or equivalent experience inbehavioral healthsettings).
• Knowledgeofmental health and addiction treatmentprinciples.
• Strongorganizational skillswith the ability to managea caseloadand deadlines.
• Exceptionalcommunication and interpersonal skills, with cultural competence to work effectively with diverse populations.
• Ability to work independently and collaboratively within ateam-oriented environment.

Additional Requirements:

• Act 33, Act 34, andAllegheny County Jail Security Clearance(requiredupon hire).
• ValidPennsylvania driver’s licenseand access to a personal vehicle with current insurance coverage.
