Job summary

Rehab Without Walls Neuro Rehabilitation in San Jose, CA seeks a Speech-Language Pathologist (SLP) for a full-time role guiding patients from evaluation to discharge in a home and community-based neurorehabilitation program.

You will serve as the primary therapist, managing a 40-hour salaried caseload (about 5–7 patients weekly) and delivering 1:1 therapy in homes, workplaces, and community settings. Rice, yoga, driving return, and work transitions feature in real-life care.

Qualifications

• Master's degree from an accredited Speech Language Pathology program.
• Valid Speech Language Pathology license in the state(s) of practice.
• Current CPR Certification.
• Minimum of one year work experience as a Speech Language Pathologist.
• Knowledge of rehabilitation techniques related to neurological injury preferred.
• Effective and professional communication in verbal and written form.

Responsibilities

• Perform evaluations and develop treatment plans for speech, language, cognitive and swallowing disorders.
• Regularly reassess effectiveness of treatment plans and attend staffing meetings and family conferences.
• Perform discharge evaluations and provide home/community safety recommendations and resources.
• Communicate patient needs and progress to the treatment team, physician, patient and family.
• Collaborate with other disciplines to ensure coordinated care and improve outcomes.
