Job summary

Luminis Health in Maryland seeks a Transitional Care - Lead RN to supervise the Transitional Care RN function and support the department manager with scheduling, evaluations, and staff development. The role designs and implements care transitions for vulnerable patients, aiming to maintain health and independence.

You will collaborate with inpatient/outpatient teams, families, and community partners to build networks that support targeted patients across care settings and coordinate treatment

Qualifications

• BSN preferred.
• Minimum 3 years of nursing experience in general medicine, ideally including inpatient and outpatient settings.
• Maryland Board of Nursing license required; CPR/BLS as relevant.

Responsibilities

• Supervise Transitional Care RN function and assist with scheduling, evaluations, and staff development.
• Design, implement, and foster care transition processes for vulnerable patients across settings.
• Collaborate with inpatient/outpatient clinicians, care coordinators, ED, families, and community partners to form a support network.
