Job summary

Avir Vista in Comanche, Texas is seeking a Charge Nurse LVN or RN to deliver high-quality, personalized care in a warm, supportive environment.

You will provide direct patient care, develop care plans, monitor status, educate patients and families, maintain HIPAA-compliant records, and collaborate with the rehabilitation team. Graduation from an LVN/RN program and Texas licensure are required, along with the ability to lift up to 50 pounds and strong communication skills.

Qualifications

• Graduation from an accredited Licensed Vocational Nursing or Registered Nursing program.
• Current LVN or RN licensure in the state of Texas.
• Ability to lift, push, and pull up to 50 pounds.
• Strong communication and interpersonal skills.
• Ability to work in a fast-paced environment, prioritizing multiple tasks and responsibilities.

Responsibilities

• Provide direct patient care to our patients, including administering medications, wound care, and other nursing procedures.
• Develop and implement individualized care plans, collaborating with our interdisciplinary team to ensure patient-centered care.
• Monitor and report patient status, identifying and addressing any changes or concerns.
• Educate patients and their families on disease management, treatment options, and preventive care.
• Maintain accurate and up-to-date patient records, adhering to HIPAA standards.
• Participate in quality improvement initiatives to enhance patient outcomes and staff satisfaction.
• Collaborate with our rehabilitation team to develop and implement individualized rehabilitation plans.
