Signature HealthCARE is seeking an experienced registered nurse at Fountain Circle Care & Rehab Center to provide direct nursing care, manage medications, and supervise staff. The role emphasizes patient safety, documentation, and collaboration with the interdisciplinary team.

The ideal candidate holds an active RN license in the practice state, has 1+ year of clinical nursing experience (long-term care preferred), and may have CPR/BLS certification.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems and MS Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during the shift.
• Monitor residents' status and notify physician of changes.
• Develop, implement, and review resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
