Job summary

Azalea Homecare is seeking a Registered Nurse to serve as Lead RN / Home Care Clinical Supervisor in Lawrenceville, GA. This part-time leadership role focuses on assessments, care planning, nursing oversight, caregiver support, compliance, documentation, and care coordination.

Responsibilities include timely acuity assessments, initial nursing assessments, developing care plans, supervising nursing staff, and coordinating with families and case managers.

Qualifications

• Active, unrestricted RN license in Georgia.
• Grad of accredited nursing program.
• Current CPR/BLS certification.
• Strong clinical judgment and communication skills.

Responsibilities

• Perform client acuity assessments within 2 business days of referral.
• Develop, review, and update individualized care plans.
• Provide RN oversight of nursing personnel and review clinical documentation.
• Coordinate with families, caregivers, case managers, and internal teams.
