The ideal candidate will hold a CA RN license, have geriatric and managed care experience, and possess a BLS certification. AltaMed offers a starting salary of $104,000 annually with comprehensive benefits.

Qualifications

• Current valid Registered Nurse license issued by the California Board of Registered Nursing.
• Minimum 1 year of geriatric nursing experience preferred.
• Minimum 1 year of managed care/case management experience preferred in ambulatory or inpatient settings.

Responsibilities

• Manage all PACE participants in the ambulatory setting.
• Lead case coordination and team leadership.
• Collaborate with physicians, social workers, and care teams to optimize patient outcomes.
