AltaMed Health Services Corp. is seeking a Registered Nurse to lead the Case Manager & Care Coordination Team for PACE participants in an ambulatory setting. This in-person role requires an active RN license in California and BLS, with preferred geriatric and managed care experience.

The position manages care coordination and case management activities, supporting high-quality patient care and coordination across services within AltaMed’s network.

Qualifications

• Current valid License as a Registered Nurse through the California Board of Registered Nursing.
• A minimum of 1 year of experience in Geriatric Nursing is preferred.
• A minimum of 1 year of experience is preferred in a managed care setting, preferably in utilization or case management in an ambulatory or inpatient setting.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Responsibilities

• The nurse (RN) case manager/care coordination team leader has the responsibility to manage all PACE participants in the ambulatory setting.
