Job summary

University Hospitals is seeking an RN Care Manager Advanced Primary Care to join the ambulatory care team in Ohio. The role focuses on care coordination, chronic disease management, and transitions of care with a strong emphasis on patient education and PHI protection.

You will collaborate with APC leadership, support advanced care planning, and work across the continuum of care to improve outcomes and patient experience.

Qualifications

• Graduate of an accredited school of nursing (Required).
• RN Licensure in Ohio and/or compact license (Required upon Hire).
• Experience with EMR applications and computer-based patient management tools (Required).

Responsibilities

• Apply nursing process for assessment, planning, delivery, and evaluation of patient care.
• Facilitate communication between patients and Primary Care Providers.
• Lead and mentor staff, delegating duties per professional standards.
