Job summary

Trilogy Health Services is seeking an RN to provide direct, quality clinical care and serve as a Team Leader for patient care staff at Wellbrooke of Wabash in Indiana. The role emphasizes supervising medications, treatments, documentation, and family education while ensuring continuity of care from admission through discharge.

The successful candidate will hold an active RN license, maintain CPR certification, and engage in ongoing professional development within a supportive team environment.

Qualifications

• Current, valid state RN license.
• Current, valid CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure medications and treatments are executed, with accurate documentation, family teaching, and care planning.
• Provide nursing care using the nursing process; ensure continuity of care from admission to discharge and community return.
• Visit residents daily to monitor physical and emotional status and alert charge nurse/physician/Director of Nursing of changes in condition.
• Identify resident problems or emergencies and initiate lifesaving measures if physician is unavailable.
• Perform administrative duties such as completing medical forms, reports, evaluations, and charting.
