Job summary

Roots Community Health seeks a Lead Physician, Clinical Practice & Academic Partnerships to drive enterprise-wide clinical governance, establish gold-standard practice models, and lead high-level academic and health system partnerships across Roots sites. Reporting to the Chief Medical Officer, this role may be 80-100% time with location flexibility in San Jose and/or Oakland.

Qualifications

• Active, unencumbered California license as a Physician (MD or DO).
• Board Certification in Family Medicine, Internal Medicine, or related primary care specialty.
• Graduate of an accredited Medical School (MD/DO).
• 5–7 years of clinical practice in primary care or community health settings preferred.

Responsibilities

• Direct clinical governance and policy development across Roots clinics.
• Lead Roots Faculty Practice and manage preceptor workflows and patient care.
• Provide direct patient care 2–3 days per week within Faculty Practice sessions.
• Pilot and refine EHR templates, care models, and protocols before enterprise rollout.
