Main duties of the job

This post is 50/50 split between virtual ward and community wards

In this role you will:

Lead, manage and develop clinical pharmacy and medicines optimisation service to virtual and community wards
Provide regular, direct, patient facing clinical pharmacy services within virtual wards and other clinical areas
Provide medicines related training to healthcare professionals
Participate and contribute to the relevant governance, quality and safety meetings, and help deliver the medicines safety agenda within virtual /community wards and pharmacy services
Liaise with primary care to ensure seamless transfer of care for patients
