You will oversee dispensing, ensure accuracy, supervise staff, manage inventory, and conduct medication reconciliation for transitions between SNF and acute care, with opportunities for PIC duties for the right candidate.

Qualifications

• Active California Pharmacist license in good standing.
• Experience in a closed-door LTC pharmacy is required.
• Proficiency with FrameworkLTC software is highly preferred.
• Strong understanding of LTC regulatory requirements.

Responsibilities

• Oversee dispensing of medications for LTC patients in compliance with state and federal regulations.
• Ensure accuracy in prescription processing and medication management.
• Collaborate with healthcare providers and facility staff to optimize patient care.
• Supervise and support pharmacy technicians and staff.
• Assist in inventory management and quality assurance programs.
• Conduct medication reconciliation for transitions between SNF and acute care.
• Maintain records of dispensing and patient interactions in compliance with regulations.
• Lead and manage pharmacy workflow; develop and implement policies (PIC duties).
