Lead Personal Care Assistant

Location: Coatesville PA

Status: Full time

Shift: Monday - Friday, 8:00 AM - 4:30 PM and every other weekend

Position Purpose:

Responsible for delivering personal care and support to participants, ensuring their health and well-being. Assists the department manager with scheduling and staffing and provides guidance and support to team members as needed.

Position Purpose:

Position will work in the facility as well as completing home care assignments.

What you will do:

  • Supporting and guiding fellow colleagues and orienting new team members

  • Assisting the department manager with scheduling and staffing

  • Escorting participants to medical and therapy appointments

  • Monitoring and reporting changes in participants’ condition or behavior

  • Ensuring compliance with infection control, safety, and organizational policies

  • Maintaining timely and accurate documentation

  • Participating in care planning and staff meetings

  • Assisting with loading/unloading of participants in transport vehicles

Minimum Qualifications:

  • High School Diploma or equivalent required

  • CNA certification preferred

  • Supervisory experience preferred

  • Valid driver’s license, insurance, and personal vehicle required for home visits

  • Minimum 1 year of experience with frail or elderly individuals preferred; training provided if less onsite after hire

  • Excellent organizational and customer service skills

  • Strong communication, critical thinking, and problem-solving skills

  • Ability to adapt to change, manage and be flexible with work priorities, and pay attention to detail

  • Proficient in Microsoft Office; comfortable with electronic communication and documentation systems
