Job description

Lead Patient Care Coordinator

This role performs the duties of a Patient Care Coordinator and serves as the first line supervisor for clinic staff. The coordinator promotes a patient‑first ethic, a culture of safety, staff engagement, and process improvement (e.g., Lean initiatives).

Responsibilities:

  1. Rooms patients and documents chief complaint, vital signs, and updates the patient’s health record in Epic.
  2. Relays physician’s instructions as directed.
  3. Processes prescription refills, form completion, patient messages, etc. in Epic.
  4. Processes labs collected in office.
  5. Administers injections as ordered by physicians.
  6. Assists physicians with clinical procedures.
  7. Obtains physician’s authorization for diagnostic tests and referrals, procedures, and medication.
  8. Reschedules patient appointments as necessary.
  9. Assists in Medical Records with scanning and filing.
  10. Prepares labels and sends urine, biopsy, and culture specimens to appropriate laboratories.
  11. Completes work queues as assigned.
  12. Assists with Quality Assurance practices, including monthly checks of the Log Binder for:
    1. Refrigerator temperature
    2. Freezer temperature
    3. Crash cart checklist
    4. Auto Clave
    5. Quality control on machines including licensing renewal and maintenance
  13. Complies with established organizational policies and procedures of WHMG.
Lead Responsibilities
  1. Makes daily rounds and provides feedback to staff on improvement areas and recognition where warranted.
  2. Consults on any questions or concerns that may arise with respect to patient care.
  3. Consults with management on personnel issues of direct reports.
  4. Promotes teamwork and provides leadership and direction to staff.
  5. Delegates work assignments to staff, including:
    1. Checking expiration dates and logging all medications
    2. Prepping and cleaning exam rooms
    3. Inventory/stocking supplies
    4. Ensuring quality assurance and safety protocols
  6. Assists in writing performance evaluations.
  7. Participates in the hiring process including screening resumes, conducting phone interviews, being part of the interview team, checking references.
  8. Oversees the training and competency sign‑off of new staff and ensures proper protocols and policies are followed.
  9. Assists in ongoing training and documentation for existing staff.
  10. Participates in and potentially leads staff meetings and huddles.
  11. Shares responsibility for QA programs, including chart audits, utilization of resources, log book maintenance and equipment checks.
  12. Utilizes performance standards to assure uniform compliance and competency in clinical skills.
Scheduling & Supplies
  1. Assisting in staffing and scheduling.
  2. Keeps abreast of staff availability and encourages flexibility of covering shifts.
  3. Collaborates with management on staffing concerns and problems.
Supplies
  1. Tracking of inventory and supplies including:
    1. Medical supplies
    2. Participates in clinic 5S
    3. Ensures sufficient exam room and clinic inventory and prepares exam room.
Community Relations
  1. Assists with educational programs.
  2. Promotes patient surveys to encourage feedback on services.
