Job summary

Monogram Health seeks a Program Manager, Specialty Programs to lead day-to-day operations for one or more specialty care programs. You will plan workflows, own scheduling, and support clinical teams across cardiology, pulmonary, endocrinology, and more to ensure seamless in-home care.

Reporting to Product & Strategy leadership, you will drive program metrics, maintain systems, and collaborate with IT, data analytics, and clinicians to scale high-quality patient care in a fast-paced environment.

Qualifications

• Bachelor's degree required; advanced degree (MBA/MPH/MHA) preferred.
• Minimum of three (3) years of healthcare operations or clinical program management experience.
• Demonstrated ability to manage complex, multi-workstream programs end-to-end.
• Advanced project management experience with multiple concurrent tasks.
• Experience leveraging AI tools (Claude) is a plus.
• Ability to translate goals into structured workplans with clear ownership.

Position: Program Manager, Specialty Programs

Monogram Health is seeking a Program Manager, Specialty Programs to serve as the day-to-day operational lead of one or more of Monogram's specialty care programs. Reporting to Product & Strategy leadership, this role plans, manages, and improves the day-to-day workflows, scheduling infrastructure, clinician support systems, and daily operational continuity that allow Monogram's clinical teams to deliver high-quality, in-home care to complex polychronic patients across cardiology, pulmonary, endocrine, palliative, behavioral health, and beyond. This is a high-ownership individual contributor role that may be asked to flex across Monogram's specialty portfolio as organizational needs evolve. The right candidate thrives in ambiguity, can hold multiple operational threads simultaneously, takes pride in building durable systems, and brings the urgency and follow-through to keep complex programs running for a seamless patient and clinician experience.

Responsibilities

• Plan, manage, and continuously improve the operational workflows of assigned specialty programs, ensuring workflows are clearly defined, consistently executed, and scalable — with a strong commitment to timely, high-quality, patient-centric service delivery
• Own clinician scheduling infrastructure for assigned specialty programs; manage day-to-day scheduling operations, monitor utilization patterns, identify capacity gaps, and drive scheduling adjustments to maintain operational continuity and visit volume targets
• Serve as the primary operational point of contact for assigned clinical teams; manage day-to-day team support needs, maintain program and clinician-level operational integrity, and ensure accurate and up-to-date records across relevant systems and workflows
• Monitor program-level and clinician-level performance metrics; surface patterns, flag risks, and elevate issues requiring leadership attention with clear framing and recommended action
• Support interviewing, onboarding, and training clinical staff as programs scale; maintain onboarding documentation and ensure new team members are set up for operational success
• Own and maintain program knowledge management infrastructure, including best practices documentation, operational playbooks, and job aids; support migration to scalable platforms as bandwidth allows
• Translate program leadership priorities into structured workplans with clear tasks, timelines, and ownership; proactively manage dependencies and follow up consistently to keep deliverables on track
• Partner cross-functionally with teams across the organization — including IT, data analytics, clinical operations, and others — to drive alignment and support successful program execution
• Collaborate with Reporting and Analytics team to develop and maintain operational dashboards and reporting in Salesforce and PowerBI; update clinician-facing tools as fields, workflows, or program requirements change
• Create clear, executive-ready materials to support regular program updates and leadership reporting
• Monitor operational KPIs for assigned program(s); use data to drive recommendations for process improvement
• Identify and close gaps in program documentation, reporting, and workflow coverage as program(s) evolve and scale
• Leverage AI tools (e.g., Claude) to enhance operational productivity, automate repeatable tasks, and improve documentation quality and turnaround

Position Requirements

• Bachelor's degree required; advanced degree (MBA, MPH, MHA) preferred
• Minimum of three (3) years of relevant experience in healthcare operations, clinical program management, or healthcare consulting
• Demonstrated ability to manage complex, multi-workstream programs end-to-end, including planning, cross-functional accountability, and escalation management
• Advanced project management experience — comfortable holding multiple concurrent workflows with a high degree of independence and without dropping detail
• Demonstrated experience leveraging AI tools (e.g., Claude)
• Able to translate program goals into structured workplans with clear tasks, timelines, and expectations across stakeholders
• Comfortable working across clinical and non-clinical stakeholders; able to communicate effectively with RNs, APPs, physicians, and senior leaders
• Thrives in a fast-paced, ambiguous environment with competing priorities and tight timelines; proactively identifies solutions and drives forward momentum
• Experience in palliative care, hospice, or complex chronic condition management preferred
• Experience in a high-growth healthcare environment strongly preferred
• Proficiency in Salesforce strongly preferred
• Experience with PowerBI and Excel strongly preferred
