Mid Cheshire Hospitals NHS Foundation Trust is seeking a Specialist Palliative Care Clinical Nurse Specialist (Band 7) to join the Community Specialist Palliative Care team in Crewe. The CNS will deliver expert assessment, care planning and symptom management for patients with cancer and other life-limiting conditions.

Aufgaben

• Lead in specialist area of practice, providing expert professional advice to others across the Trust within guidelines and protocols.
• Perform comprehensive specialist assessment of community patients, plan, treat and evaluate care, including prescribing where appropriate.
• Communicate complex specialist information to generalist colleagues and refer patients to appropriate clinicians as needed.
• Prioritise patient care, supervising workload, monitoring and evaluating effectiveness, including discharge or referral within guidelines.
• Perform specialist clinical skills in treatment and overall management of patient care, including investigations.
• Provide a comprehensive specialist service, accepting accountability for the community service day-to-day.
• Collect, collate, evaluate and report information, maintaining accurate patient records and documentation.
• Promote patient and public involvement in service improvement and involve carers in planning and delivery of care.
• Work collaboratively with other professionals to ensure patient needs are met, including patient flow and discharge planning.
• Analyse information to proactively manage NHS targets and standards.
