Job summary

Linley Park Post Acute in Anderson, SC is seeking a licensed Occupational Therapist (OTR) to deliver patient-centered therapy in a skilled nursing setting. The role involves evaluation, treatment planning, and collaboration with an in-house therapy team to improve residents' independence and quality of life.

You will supervise COTAs, aides, and students, document progress in accordance with regulations, and contribute to care planning while providing high-quality therapy aligned with physician

Qualifications

• Degree in Occupational Therapy (Bachelor’s required; Master’s or Doctorate preferred).
• Active South Carolina Occupational Therapist (OTR) license in good standing.
• Experience in skilled nursing or rehab setting preferred.
• Familiarity with PointClickCare and Casamba preferred.
• Strong clinical, documentation, and leadership skills.

Responsibilities

• Evaluate residents within required timeframes and develop individualized treatment plans.
• Provide therapy services per physician orders and established goals.
• Document evaluations, treatments, and progress notes in compliance with regulations.
• Supervise COTAs, aides, and students in accordance with state practice act.
• Collaborate with the interdisciplinary team on care planning and patient progress.
• Participate in care conferences, utilization reviews, and rehab meetings.
• Educate residents, families, and caregivers on therapy programs and discharge plans.
• Assist with durable medical equipment (DME) recommendations.
• Provide staff training and participate in in-services.
• Ensure compliance with all state, federal, and payer requirements.

Education

Bachelor’s degree in Occupational Therapy

Master’s degree or Doctorate preferred
