Job summary

Freedmen’s Medicine in Washington, DC is seeking a Board-Certified Nurse Practitioner to deliver primary and transitional care to chronically ill patients in home, facility, and other settings within a hybrid model.

The role requires 1–3 years NP experience with chronic diseases, DC/MD licenses, DEA, BLS and ACLS, and the ability to coordinate care, meet deadlines, and collaborate with Medical Director and Operations.

Qualifications

• Board-certified NP with chronic care management experience.
• DC and MD licenses required; DEA number.
• BLS and ACLS certifications required.
• Experience with high-need patients and care transitions.
• Ability to manage complex care plans and documentation within 48 hours.

Responsibilities

• Deliver primary care and comprehensive health assessments in home and facilities.
• Manage an assigned panel of complex patients within a value-based care model.
• Review and sign off on clinical care plans and hospital discharge reviews.
• Lead multi-disciplinary teams and coordinate care with the Medical Director.
• Document all clinical notes within 48 hours in the EMR.
• Adhere to federal and state patient safety guidelines including medication reconciliation.
