  • Provide comprehensive nephrology care to patients with acute and chronic kidney diseases at home and in dialysis facilities
  • Collaborate with interdisciplinary teams to develop and implement value-based care initiatives aimed at improving patient outcomes and reducing healthcare costs
  • Utilize evidence-based practices and clinical guidelines to optimize management of kidney disease and comorbidities
  • Engage in patient education and shared decision-making to empower patients in managing their kidney health
  • Utilize technology and data analytics to monitor patient outcomes, identify improvement areas, and drive quality initiatives
  • Participate in multidisciplinary care conferences to review complex cases and collaborate on treatment plans
  • Drive clinic metrics impacting kidney care, such as Home Dialysis rate, access, dialysis start, and conservative management

Position Requirements

  • Medical degree from an accredited medical school
  • Board certification in Internal Medicine and Nephrology
  • Current state medical license and DEA registration without restrictions
  • Strong clinical skills and experience managing kidney disease patients across settings
  • Commitment to patient-centered care and shared decision-making
  • Excellent communication and interpersonal skills for effective collaboration
  • Experience with value-based care, quality improvement, and population health preferred
