Job summary

Qualifications

• Must be certified Medication Aide and meet state requirements.
• At least 1 year of experience with elders or disabled individuals.
• Demonstrated leadership capabilities.
• Knowledge of Assisted Living or Dementia care preferred.
• Ability to work weekends, evenings and flexible hours.

Responsibilities

• Monitor residents' health and well-being and lead care activities.
• Assist with medication administration and care coordination.
• Document monthly wellness visits and PSP updates.
• Communicate changes to Wellness Director and Executive Director.
• Support wellness visits and uphold clinical quality assurance.
