Qualifications

• Graduation from an accredited Nursing School.
• Valid LVN/LPN license.
• Ability to observe and report patient conditions.
• Document patient assessments and changes in EMR as required by policy.

Responsibilities

• Participate in and oversee patient care within the LVN/LPN scope of practice.
• Observe, assess, report patient conditions and document per policies and regulations.
• EMR documentation related to patient assessment and changes in condition.
• Ensure care plans are implemented and medications/treatments administered as prescribed.
• Supervise CNA staff to ensure high-quality care and patient well-being.
