Job summary

StoryPoint Group in Green Bay is seeking an experienced Licensed Practical Nurse (LPN) to supervise a team of caregivers at StoryPoint Howard. The role focuses on resident evaluation, care planning, and coordination with physicians using EHR/EMAR.

Qualifications

• Must hold an active LPN license in the state.
• Supervisory experience required.
• Assisted Living/Memory Care experience preferred.
• Excellent punctuality and professionalism.
• Passionate about working with the elderly.

Responsibilities

• Lead, manage, and train your team of caregivers.
• Evaluate residents and report changes in condition to physicians.
• Utilize Electronic Health Records (EHR) and Electronic Medication Administration Records (EMAR).
• Communicate effectively with team, residents and families.
• Build strong relationships with residents and staff.
