Job summary

Folkston Park Care and Rehab is hiring a Licensed Practical Nurse (LPN) to work in short-term rehabilitation and long-term care settings. The role focuses on implementing individualized care plans and supporting patient wellbeing.

Under the Unit Director, the LPN will assist in orienting and supervising nursing staff, manage per-shift unit operations, and assume leadership responsibilities as needed.

Qualifications

• Graduate of an approved school for Practical or Vocational Nursing.
• Currently licensed by the State Board of Nursing.
• Experience in geriatric or rehabilitative nursing preferred.

Responsibilities

• Assist in development and implementation of individualized Nursing Care Plans.
• Supervise nursing personnel and participate in unit operations per shift.
• Perform leadership duties as needed under the Unit Director.
