Job summary

The Veterans Health Administration’s Daytona Beach VA Clinic seeks an experienced Licensed Practical Nurse to perform complex nursing care in the outpatient setting under RN supervision.

The PACT LPN will administer medications, monitor patient status, document outcomes, and collaborate with an interprofessional team to support veterans' health plans.

Ideal candidates hold active LPN license, English proficiency, and a commitment to high-quality patient care.

Qualifications

• Graduate of a school of practical nursing approved by the state agency or NLNAC accreditation.
• Active, unrestricted licensure as a licensed practical or vocational nurse in the U.S. or D.C.
• English language proficiency for direct patient care as required by VA standards.

Responsibilities

• Performs complex LPN/LVN nursing care under RN supervision.
• Administers medications and treatments per policy; observes and reports patient status.
• Documents care provided and patient responses to care plans; assists in care plan development.
• Supports Veterans in achieving optimal health outcomes through patient education and coordination with the care team.
