Job summary

The Village At Germantown is hiring a Staff Nurse to provide direct nursing care to residents across Skilled Nursing and related units in Germantown, TN. You will supervise nursing assistants, administer medications, perform treatments, and complete required documentation.

You must hold a Tennessee or multi-state LPN license, have at least two years of LPN experience (geriatric/medical-surgical preferred), and demonstrate strong communication and leadership.

Qualifications

• Licensed Practical Nurse with a current State of TENNESSEE or Multi-State License.
• Must read regulations and patient records and write clearly.
• Must communicate in a courteous, friendly manner.
• Ability to manage and direct subordinates.
• Physical requirements attached.

Responsibilities

• Provides daily supervision and nursing care to residents in multiple units.
• Walks rounds with outgoing LPN and reports to ensure proper shift communication.
• Assesses residents each shift and acts on changes in condition.
• Assists with admissions and completes assessments and orders.
• Supports ADLs and promotes independence for residents.
• Responds promptly to call system and meets resident needs.
• Identifies medical/behavioral changes and updates care plans.
• Provides daily reports to nursing leadership on conditions and needs.
• Completes tasks on schedule; overtime only with approval.
• Demonstrates hospitality and upholds core values.
• Ensures weights are entered per policy.
• Monitors meal service for resident safety.
• Ensures grooming, showers and oral care for residents.
• Delivers discharge instructions and medications on discharge.
• Completes incident reports and audits timely.
