Job summary

Qualifications

• Must hold NC LPN license in good standing.
• Minimum 2+ years of experience in a long-term care setting.
• Strong leadership and management skills.
• Excellent communication and interpersonal skills.
• Ability to lift, transfer and reposition patients as needed.

Responsibilities

• Lead a team of licensed practical nurses (LPNs) and CNAs to ensure seamless delivery of patient care
• Develop and implement care plans in collaboration with the interdisciplinary team
• Monitor patient progress and identify areas for improvement, making necessary adjustments to care plans
• Provide guidance and support to LPNs and CNAs to promote best practices in patient care
• Participate in quality improvement initiatives to optimize patient outcomes
• Collaborate with other departments to ensure smooth transitions of care
• Maintain accurate and timely records of patient care and progress
• Provide education and training to staff members to enhance their knowledge and skills
