Job summary

StoryPoint Medina East is seeking a Licensed Practical Nurse (LPN) to supervise and support a compassionate caregiving team serving seniors in a dynamic assisted living environment.

You will lead, train, and evaluate staff, ensure high-quality resident care, and collaborate with physicians, families, and the care team to monitor residents’ conditions and document changes using EHR/EMAR systems.

Qualifications

• Licensed Practical Nurse (LPN) license in the state of Ohio
• Assisted Living/Memory Care experience preferred
• Prior supervisory experience required
• Punctuality and professionalism are important factors

Responsibilities

• Lead, manage, and train your team of caregivers.
• Responsible for evaluating residents and reporting changes in condition to physicians.
• Utilizing Electronic Health Record (EHR) and Electronic Medication Administration Record (EMAR).
• Communicating effectively to team, residents and families.
• Building strong relationships with residents, families, and staff
