Job summary

Montefiore Einstein Advanced Care is seeking a Lead LPN to support the clinical team across multiple practices in New York. The role includes recording vital signs, patient history, and exercising nursing responsibilities while assisting with scheduling and authorizations.

You will supervise clinical associates and drive efficient daily operations under the Practice Manager. The position requires LPN licensure in New York, 4–6 years of clinical experience, and the ability to train staff.

Qualifications

• High School diploma or equivalent required.
• Associate degree (AS) preferred.
• 4–6 years of nursing or related clinical experience.
• 1–5 years relevant to tier.
• Annual Assessment - OHS required.
• New York LPN license required by NY Board of Nursing.
• NCLEX-PN or NYSED-acceptable license required.

Responsibilities

• Administers medications, injections, insulin, and immunizations.
• Monitors and records vital signs and patient history.
• Documents intake/output and maintains accurate records.
• Assists with scheduling and authorizations to support admin staff.
• Trains and supervises clinical associates on workflows.
• Maintains clean patient care areas and stocks supplies.

Job Summary In addition to their regular daily shift LPN duties, the Lead LPN will perform additional clinical and administrative tasks that will provide support to their clinical team and practice. The Lead LPN will be responsible for recording patient's vital signs, patient intake and output, and recording patient history. This associate will administer medication, injections and functions in scope of license. This position will clean equipment in patient care areas and ensure patient care rooms are kept clean and supplies are well stocked. The Lead LPN will maintain records of patient activities and observations and submit records to the appropriate clinical staff regarding patient's condition. The Lead LPN will assist with scheduling patients and authorizations to support administrative staff as needed. The Lead LPN will train and monitor all clinical associates on all related clinical and clerical workflows. The Lead LPN will advise the Practice Manager of associate performance to address any areas of training and development required. The Lead LPN will be responsible for day-to-day activities including, but not limited to, planning and development, scheduling and daily operations. The Lead LPN will ensure their clinical team works with clinical and clerical support staff to deliver excellent patient care and promote patient satisfaction. The Lead LPN will also practice under the direction of the Practice Manager. The LPN will work with clinical and clerical support staff to deliver excellent patient care and promote patient satisfaction across multiple practices. 1. Administers medications, injections, insulin, immunizations, perform finger sticks and functions in scope of license.
2. Identifies patient goals, communicates, and collaborates with clinical team to support patient care plan.
3. Performs clinical workflows.
4. Monitor and record patient's vital signs, patient history and reason for visi.t
5. Explain treatments and procedures to patients and families.
6. Assist with scheduling, referral and follow up appointments.
7. Cross cover multiple practices.
8. Assist with authorizations and administrative needs as needed.
