Job summary

Connections Health Solutions is seeking a Lead Licensed Case Manager for Night shifts in King of Prussia, PA. You will coordinate patient care across walk-in, Observation, and CSU units, ensuring timely documentation and regulatory compliance, while acting as a clinical resource for the team.

The role emphasizes recovery‑oriented care, collaboration with physicians and peers, and supporting patient flow and discharge planning. A master’s in a behavioral health field and PA licensure are required.

Qualifications

• Master's degree in a behavioral health field is required.
• PA licensure (LSW/LAC) in good standing is required or pursued.
• Clinical case management and coordination experience preferred.
• Experience with shift-based lead support and workflow coordination is valued.

Responsibilities

• Provide direct clinical case management in walk-in, observation, and CSU units.
• Coordinate care with interdisciplinary treatment team; support treatment planning and discharge.
• Maintain complete clinical documentation meeting regulatory standards.
• Provide shift-based guidance and coaching to case management staff.
• Support patient flow and continuity of care through discharge planning.
