River Edge Behavioral Health in Macon, GA is seeking a Lead Individual Support Worker to supervise the team and be available for on-call duties. You will assist clients with daily living activities, observe behavior, and train staff in basic skills within home or residential settings.

Responsibilities include leading ISP meetings, coordinating weekly home sessions, and facilitating transportation and daily living activities while maintaining safety and confidentiality.

Qualifications

• High school diploma or GED required.
• Must be at least 21 years of age.
• At least 1 year direct support experience.

Responsibilities

• Track client goals at home and document progress.
• Attend ISP meetings annually at day center.
• Conduct weekly home meetings.
• Instruct individuals in daily living skills one-on-one or in groups.
• Assist with housekeeping and errands as needed.
• Provide transportation to appointments and activities.
• Maintain medications at home.
