Job summary

Good Shepherd Hospice in Ft. Worth is seeking a compassionate Patient Care Manager to join the Leadership Team in our Ft. Worth Office.

This role coordinates all aspects of direct patient care under the Executive Director, serving as a team leader and supervising direct care staff. The position requires RN licensure, hospice experience, and at least 1 year of supervisory responsibility. You will participate in budgeting, staff development, interviews and ongoing education to maintain

Qualifications

• RN licensed in the state where care is provided.
• Minimum 1 year of hospice supervisory experience.
• Experience in hospice, home health, geriatrics or general med/surg nursing.
• Demonstrated commitment to the hospice philosophy of care.

Responsibilities

• Coordinate development, implementation and review of each patient/family plan of care.
• Lead interdisciplinary team meetings and supervise clinical staff.
• Participate in hiring, orientation and staff competency assessment.
• Act as Alternate Administrator in absence of Executive Director.
• Support budgeting and monitor patient care expenses.
• Engage with nursing home staff and external partners.
• Contribute to QAPI committees and ongoing education.
